  1. Fill out the Online Application Form and follow the instructions given (for DLSMHSI SHSSHS students, proceed to Screening and Evaluation number 1).

  1. Pay the non-refundable / non-transferable Php 500.00 (for Filipino applicants) / $125.00 (for foreign applicants) College Admission Test (CAT) Fee through Dragonpay (see link for the bank details and video for the payment procedures).  
  1. After receiving your Official Receipt from our cashier, log in to the applicant’s portal and choose your preferred Entrance Examination schedule.

Note: The online exam is available for applicants from outside Cavite only. Please send an email first to pfcampana@dlsmhsi.edu.ph to request the online exam. Kindly provide your full name and address. Slots are limited and will be allocated on a first-come, first-served basis, subject to approval. 

  1. Read and watch the written and video guidelines provided and other details for your entrance examination.
 
  • Take the "Practice Examination" at your earliest convenience (during your free time) to familiarize yourself with the general aspects of the exam. If you encounter any issues opening the software, TRAST will be available to assist and resolve the problem before your actual exam.
  • Take the Online CAT as scheduled. Applicants should be online and logged into their account at least 30 minutes before the examination. Please note that applicants are expected to have read the Guidelines on taking the DLSMHSI Online Entrance Examination and have already completed the Practice Exam.
  • A rescheduling fee of Php 300.00 shall be paid if the applicant fails to attend or wishes to reschedule their entrance examination.
  • The result of the Online CAT shall be released through the applicant’s portal after finishing the entrance examination.

 

  1. Once you’ve chosen your desired course, proceed to pay the Php600.00 Admissions Processing Fee through Dragonpay (see link for the bank details and video for the payment procedures).
  1. Log in to the applicant’s portal and choose your preferred interview schedule.
  1. Read the written guidelines provided and other details for your interview.
    • A rescheduling fee of Php 300.00 shall be paid if the applicant fails to attend or wishes to reschedule their interview schedule.
  1. Check the applicant’s portal 1-2 days after your interview or contact The Registrar - Admissions, Scholarships and Testing Section (TRASTS) for the final result of the evaluation.

  1. Secure the Acceptance Letter from TRASTS through the applicant’s portal, which contains the link for complete instructions on the next steps, should the applicant pass all the screening and evaluation criteria.
  1. Pay the Php 10,000 reservation fee through Dragonpay (see link for the bank details and video for the payment procedures) to confirm your slot. 
 
  • The reservation fee is non-refundable and non-transferable but should the applicant enroll in this Institution, the aforementioned amount shall be deducted from their total tuition fee
  • Applicants who do not comply with all the requirements regardless of payment shall have their reservation fees forfeited. 
  • Deadline for reservation of slots is one month upon receiving the Acceptance Letter. 
  • Failure to comply with the aforementioned policy would mean forfeiture of slots.

  1. Submit the following requirements through personal visit (Monday to Saturday, 8:00 AM to 4:00 PM) or via mail/courier to receive your Enrollment Permit and Enrollment Procedure in the applicant’s portal:

Requirements for applicants from DLSMHSI:

 
  • Photocopy of Birth Certificate
  • Photocopy of High School Diploma or Certificate of Graduation

Requirements (for applicants from other school):

 
  • Photocopy of Birth Certificate
  • Original Form 138 (6th year High School Report Card)
  • Photocopy of High School Diploma or Certificate of Candidacy for Graduation

Additional requirements for foreign applicants:

 
  • Photocopy of Passport 
  • Photocopy of Alien Certificate of Registration (ACR) for permanent residents, native born, children of diplomats, or holder of any valid visa and Certificate of Residence for Temporary Students (CRTS) 
  • Special Study Permit (SSP) for foreign students studying in the Philippines for less than a year or are 18 years old and below 

For applicants who cannot submit the said original documents on or before enrollment, you can submit the Original Notarized Provisional Enrollment Form for you to enroll temporarily. Once the lacking requirements are available before the specified date on your Notarized Provisional Enrollment Form, you may submit them to TRASTS through personal visit (Monday to Saturday, 8:00 AM to 4:00 PM) or through mail/courier at the address below for us to update your enrollment status.
